Online TRT programs compared, and the 89 days between blood draws

No affiliate links on this page. We earn no commission from any program listed, nobody paid to appear here, and the order is not for sale.

Testosterone therapy is run on bloodwork. A panel sets the starting protocol, and a later panel says whether it worked. In between, the programs on this page differ mostly on three things: what they charge, whether the labs are bundled or billed on top, and what they actually prescribe, because not all of them prescribe testosterone at all.

The retest cadence is the reason this page exists. A quarterly panel is the industry norm, which leaves about 89 days where you are the only person watching. Testosterone therapy is monitored for red blood cell concentration and blood pressure among other things, and those move slowly enough that a single reading tells you almost nothing and a daily log tells you a lot.

Emerge Health AI does not order labs, read labs, or adjust a protocol. It is a wellness app, not a medical device. What it does is answer your questions about what a marker means with a citation you can open, and keep the blood pressure, sleep, activity and mood record that your next appointment would otherwise run on memory alone.

These are numbered, not ranked. Each entry leads with the one thing it is factually best at. We do not score these programs on the quality of their clinical care.

Best for scheduled retesting

Hone Health

The only program here that publishes a fixed retest cadence, with 50+ biomarkers and follow-ups written into the membership rather than sold as add-ons.

  • Advanced testing for 50+ key biomarkers
  • Retesting and follow-ups every 90 days on the Premium tier
  • Private telehealth consultations with licensed physicians
  • Member pricing on medications and supplements, with no commitment and cancel anytime

What it leaves to you

  • The membership and the medication are separate bills, so the monthly figure on the page is not the monthly figure on your card.
  • The Basic tier tests every six months rather than every 90 days and sells consults individually, so the cheaper entry point is also the thinner one.
  • Ninety days between panels is a long time to be the only person watching. What happens in between is yours to notice and yours to report.
Lowest flat monthly price

TRT Nation

A single flat subscription with unlimited physician consultations, labs ordered nationwide, and medication delivered to your door.

  • One flat monthly price with no tier to choose between
  • Unlimited physician consultations rather than a per-visit charge
  • Labs ordered nationwide as part of the online intake
  • Medication delivered to your home

What it leaves to you

  • Every figure here comes from the company’s own editorial pages rather than a structured pricing page, so what is and is not inside the $99 is less clear than it looks.
  • Unlimited consultations means the initiative is yours. Nothing is scheduled on your behalf, so the follow-up happens when you ask for it.
  • If lab cost sits outside the subscription, the cheapest headline price in this comparison may not be the cheapest total.
One price including medication

Henry Meds

The same single-payment structure Henry Meds uses for its weight programs, applied to testosterone: consultation, prescription and delivery in one monthly figure.

  • One monthly payment covering the visit, the medication and the supplies
  • No insurance paperwork at any point
  • Ongoing care included rather than billed per visit
  • The same billing structure across its weight and hormone programs

What it leaves to you

  • Lab work is generally not bundled, so the bloodwork that testosterone therapy is monitored on is a separate arrangement and a separate cost.
  • No fixed retest or visit interval is published, so the monitoring rhythm is set by you rather than by the program.
  • Insurance is not involved, so the monthly cost is entirely out of pocket and not offset by coverage you may already hold.
Built around enclomiphene, not testosterone

Maximus

The one program here whose flagship protocol is not testosterone at all, but a medication intended to stimulate the body’s own production, with at-home labs included.

  • A non-injectable protocol, for people who would rather not inject
  • CLIA-certified at-home lab kits inside the price
  • Medication included rather than billed on top
  • Asynchronous messaging with the care team

What it leaves to you

  • This is a different class of medication from the rest of this page. Whether it suits you is a clinical question for a clinician who knows your history, not something a comparison table can answer.
  • Care is asynchronous, so nothing is scheduled and the initiative to raise something is yours.
  • It is the highest published monthly price in this comparison, though it bundles labs and medication that others bill separately.
Most extensive lab work

Marek Health

The functional-medicine end of this category: expanded biomarker panels and root-cause diagnostics, at the highest combined cost on the page.

  • The widest biomarker panel of any program on this page
  • Root-cause diagnostics rather than a single hormone reading
  • Coaching alongside the clinical protocol
  • Protocols spanning more than testosterone alone

What it leaves to you

  • The upfront lab cost is the largest single number in this comparison and sits outside the monthly fee, so the first month is far more expensive than the steady state.
  • A wider panel produces more numbers, and more numbers produce more questions. Most of those questions will occur to you between appointments.
  • Every figure here is third-party sourced, because the company does not publish its pricing anywhere we could read directly.

Common questions

Can Emerge Health AI order or interpret my blood work?

No. Emerge does not order labs, read lab results, or adjust a protocol, and it does not store lab panels. It answers general questions about what a marker means, with citations to sources like MedlinePlus, NIH and Mayo Clinic that you can open and read yourself. Interpreting your results is your clinician’s job.

How often do online TRT programs retest?

A quarterly panel is the common pattern, and Hone Health is the program here that publishes it explicitly: retesting and follow-ups every 90 days on its Premium tier. Most others do not publish a fixed interval, which means the monitoring rhythm is set by you.

Do these programs include the cost of lab work?

It varies, and it is the biggest hidden difference between them. Hone Health and Maximus bundle testing into the membership. Marek Health bills lab work on top, with reported upfront costs of roughly $450 to $1,700. Henry Meds and TRT Nation do not publish labs as bundled.

What can I track between panels?

Blood pressure with staging, sleep, activity, steps, your dose and injection days, and a private journal for energy and mood. Testosterone therapy is monitored partly for effects on red blood cell concentration and blood pressure, and those move slowly enough that a trend across weeks says more than any single reading.

Do all of these programs actually prescribe testosterone?

No, and it is the most important distinction on the page. Maximus leads with enclomiphene, a different class of medication intended to stimulate the body’s own production rather than replace it. Whether that suits you is a clinical question for a clinician who knows your history.

How we compare

Programs are compared only on facts that can be verified in public: what they prescribe, what they charge, whether lab work is bundled into the price, and how often a clinician reviews you.

We do not rank these programs on the quality of their clinical care, and we do not compare protocols on effectiveness. Neither is a claim we are equipped to make or willing to publish.

Every figure is marked on the provider’s own page here for how it was checked. A filled marker means it was read off that company’s public page. A hollow marker means it came from a dated third-party review.

Pricing in this category moves, and aggregator sites are frequently out of date: one widely repeated figure for Hone Health was $149 when the company’s own page said $155. Everything here was checked in the month shown and re-checked quarterly. Corrections to hello@getemergeapp.com.

Where Emerge fits, on any of them

Emerge cannot order a panel, interpret your results, or change a protocol, and it will say so when asked. It is the record and the reading between appointments.

  • Ask what a marker on your panel means and get an answer citing MedlinePlus, NIH or Mayo Clinic, with the link, so you can read the source rather than a forum thread.
  • Track blood pressure with staging. Testosterone therapy is monitored partly for effects on red blood cell concentration and blood pressure, and a trend over weeks is the thing a quarterly appointment cannot see.
  • Log every dose and every injection day, so the protocol you are actually on is documented rather than remembered.
  • Track sleep, activity and steps, plus a private journal for mood and energy, which are the subjective changes people most want to discuss and least reliably recall.
  • All of it encrypted on your phone, on a system that keeps no conversation logs.
