Online psychiatry compared, and the 29 days between appointments

No affiliate links on this page. We earn no commission from any program listed, nobody paid to appear here, and the order is not for sale.

The decisive difference between these services is not clinical, it is financial. Talkiatry and Headway require active insurance and publish no self-pay route. Brightside and Rula publish cash rates alongside insurance. Which of those applies to you determines what is even available before any question of fit arises.

The reason this page exists is the shape of the care. Medication management typically runs on a monthly or longer cycle, and a follow-up appointment is often fifteen minutes. Those fifteen minutes usually open with how has your sleep been, did you take it every day, and how have you actually felt, which are three questions about the twenty-nine days nobody was recording.

Emerge Health AI is not therapy, not a crisis service, and not a mental health provider. It is a wellness app. What it can do is hold the sleep, activity, adherence and journal record that those fifteen minutes run on, and answer medication questions with sources you can open.

These are numbered, not ranked. Each entry leads with the one thing it is factually best at. We do not score these programs on the quality of their clinical care.

Lowest published cash psychiatry rate

Brightside Health

The only service here with a published monthly cash price for psychiatry alone, alongside separate therapy and bundled plans.

  • A published cash price, so the cost is knowable without an insurance check
  • Psychiatry available on its own rather than bundled by default
  • A discounted combined plan if you want both
  • In-network coverage with most major insurers

What it leaves to you

  • The bundled plan costs less than therapy alone, which means the pricing is built to move you toward the bundle rather than the cheaper single service.
  • Cash therapy at about $399 a month is the highest recurring figure on this page.
  • A monthly subscription implies a monthly rhythm. Whatever happens in the weeks between is yours to notice and to raise.
Insurance-only, no cash route

Talkiatry

In-network psychiatry across 100+ insurers, with no self-pay option at all, which makes it either very cheap or entirely unavailable.

  • Among the lowest out-of-pocket costs available if you are commercially insured
  • Over 100 major insurers in network
  • Psychiatrist-led medication management rather than a general practitioner route
  • Therapy available alongside medication management

What it leaves to you

  • If you are uninsured or underinsured, this service is simply not available to you. There is no cash route.
  • Therapy cannot be used on its own here, so it is not an option if medication is not part of what you want.
  • A copay-based model means your cost depends entirely on your plan, and is not knowable from this page.
Published cash rates for both services

Rula

Per-session pricing rather than a subscription, with published cash rates for therapy and psychiatry and a large insurance network behind them.

  • Therapy and psychiatry each available on their own
  • Published cash rates, so the cost is knowable without insurance
  • Per-session billing rather than a monthly subscription
  • A large insurance network behind the same service

What it leaves to you

  • Per-session pricing means the monthly cost depends entirely on how often you go, which is not knowable in advance.
  • The gap between the cash rate and the insured rate is roughly tenfold, so coverage matters more here than almost anywhere else on this page.
  • Nothing is scheduled on your behalf between sessions.
Lowest insured per-session cost

Headway

A network that matches you to in-network clinicians, with the lowest reported insured session cost here and no published self-pay route.

  • The lowest reported insured cost per session on this page
  • Both therapy and medication management through one network
  • Matching to clinicians who are already in network
  • No subscription layered on top of the session cost

What it leaves to you

  • Without active insurance you cannot sign up at all, and no self-pay rate is published.
  • Because it is a matching network rather than a single practice, the experience depends on the individual clinician you are matched with.
  • Every figure here comes from a competitor’s comparison page rather than a published rate card, which is a weaker source than we would like.

Common questions

Is Emerge Health AI a substitute for therapy or a crisis line?

No, and it is important to be plain about this. Emerge is a wellness app, not a medical device and not a mental health service. It does not provide therapy, does not diagnose, and does not prescribe. If you are in crisis, contact your local emergency number or a crisis line: in the US, call or text 988 for the Suicide and Crisis Lifeline.

What is the real difference between these services?

How you pay. Talkiatry and Headway require active insurance and publish no self-pay route at all. Brightside and Rula publish cash rates alongside insurance. That single fact decides whether a service is available to you before anything clinical enters the picture.

What is worth tracking between psychiatry appointments?

Sleep, activity, and which doses you actually took and when, plus a private note on how each week felt. A medication-management follow-up is often fifteen minutes, and those are precisely the questions it opens with.

Can Emerge tell me whether a medication side effect is normal?

It can tell you what published sources say, citing openFDA, DailyMed or MedlinePlus so you can read them yourself. It cannot tell you whether to continue, stop, or change a dose. Those decisions belong to your prescriber, and Emerge will say so rather than guess.

How we compare

Services are compared only on facts that can be verified in public: whether self-pay is possible, what the published rates are, whether insurance is required, and what a subscription covers.

We do not rate the quality of anyone’s clinical care, and we are especially unwilling to do so in this category. Nothing here is a recommendation about treatment.

Every figure is marked for how it was checked. A filled marker means it was read off that company’s public page. A hollow marker means it came from a dated third-party review.

If you are in crisis, this page is not the right resource. In the US, call or text 988 for the Suicide and Crisis Lifeline. Corrections to hello@getemergeapp.com.

Where Emerge fits, and where it does not

Emerge is not therapy, not a crisis service, and not a substitute for the clinician managing your care. It does not diagnose or prescribe, and it defers on anything clinical. What it is good at is the record between appointments.

  • Track sleep and activity continuously, because those are the first questions at a medication-management follow-up and the hardest to answer from memory.
  • Log every dose, so adherence is a record rather than an estimate, on medications where timing genuinely matters.
  • Keep a private journal of how each week actually felt, encrypted on your phone, on a system that keeps no conversation logs and shares nothing.
  • Ask what a medication side effect is reported to be and get an answer citing openFDA, DailyMed or MedlinePlus, with the link, so you read the source rather than a forum.
  • It will not tell you whether to change or stop anything. That decision belongs to your prescriber, and Emerge says so plainly rather than guessing.
